Who we are
Cancer Council Australia (“Cancer Council”, “we/us”) is collecting your personal information and health information so we can manage this referral and provide Navigation support.
What we collect and where it comes from
We collect information about you from your referring health professional (and we may also collect information directly from you). This may include your contact details, demographic details, information about your diagnosis/treatment, symptoms and support needs, and other information needed to triage and coordinate services.
Why we collect it
We collect, use and disclose your information to:
- receive, assess and manage your referral (including intake, triage and care coordination)
- contact you about the referral and provide Navigation support and related services (including follow-up about support needs connected to the referral).
If you don’t provide the information
If the requested information is not provided, we may not be able to progress the referral, contact you, or provide appropriate services or support.
Who we share it with
We may share your information with:
- relevant Cancer Council Australia staff involved in referral intake and Navigation support
- third-party service providers, including relevant State or Territory Cancer Councils, who help us run the referral process and deliver services (for example, IT systems and secure communications tools)
- other health or support services involved in your care, where needed to progress the referral or for a related purpose that you would reasonably expect your information to be used for.
Updates to your referrer (only if you agree)
If you consent, we may provide referral progress/status updates to your referring health professional. If you give a separate consent, we may share additional health information with them where required for that purpose.
How we protect your information
We take reasonable steps to protect personal information from misuse, interference and loss, and from unauthorised access, modification or disclosure.

Withdrawing consent/contact preferences
You can withdraw consent or update your contact preferences at any time by calling 13 11 20 or emailing [insert email]. If you withdraw consent, it may affect our ability to progress the referral or provide services.
